While you are not required to upload this form upon submitting payment, Oscar reserves the right to request a copy of the signed form for up to three years from payment date. IMPORTANT NOTE: It is your responsibility to ensure that your client completes the Payment Authorization and Consent form and you must maintain the signed copy for your records. Oscar can take your clients’ checking or savings account information over the phone for one-time payments, or to set up autopay. Unfortunately, we do not accept credit card payments.* Set up autopay via ACH with a checking account, or make a one-time payment with a debit card. This is the easiest, fastest, and most secure way. Pay online at or through the Oscar mobile app. Pay at the time of enrollment! For 2020 on-exchange (FFM only) and all off-exchange plans, we will accept ACH binder payments at the time of enrollment. New Jersey allows a 30-day reinstatement period for on-exchange members who do not pay by January 1st. For off-exchange members, if payment is not fully processed by 11:59 PM on January 1st, 2020, the member’s coverage will be cancelled with no option to reinstate without a qualifying life event for the remainder of the year.
